Athletic Clearance

Don't Forget - Athletic Clearance is Required

You must have your Athletic Clearance completed with the school prior to any physical participation including conditioning with lacrosse.  Visit the Woodcreek HS website for information and how to obtain your clearance. This is required by the school.

PLEASE NOTE: The school/district no longer accepts physicals completed by a Chiropractor's office.
